Is this an issue with the signal coming from the 9250? I feel like this is somehow related to Soft 15 and the card I'm using. Anybody? :banghead:

While I am FAR from the expert on PC to arcade monitor however a few things that I would try that I didnt hear in your vid:
1. Does the PC work with a "standard" PC monitor
2. Am I totally sure this is a totally functional arcade monitor
3. Is my desktop size and refresh rate set correctly
4. Do I have the correct video card drivers installed
5. Have I double checked how everything is hooked up / wired together
Again you might have tried most of this but off the top of my head in addition to what I heard in your video I would check the few things I listed as well.

Yes - If I plug in my regular PC monitor things work fine so the video card is working alright.
2. Am I totally sure this is a totally functional arcade monitor
The monitor seems to have its moments where it looks great. I have seen moments where the color and clarity of the monitor have been great, but I am concerned that my jumping picture and unstable signal are to blame for the issue.
3. Is my desktop size and refresh rate set correctly
The resolution is set to 640x480 @ 60khz in Quick Res, and I have put Soft 15 on my D:// drive, run it and seen that it does say "Active" for 15khz.
4. Do I have the correct video card drivers installed
I used the drivers on the disk to start, then downloaded and updated to the newest drivers from the site. To my knowledge these are just fine for Soft 15 use, and are working fine on my PC monitor.
5. Have I double checked how everything is hooked up / wired together
Everything is wired properly to the best of my knowledge. I'd be more concerned that this were the issue if I weren't getting a picture at all, or the problem was a little more evident of not being wired up properly.
Again you might have tried most of this but off the top of my head in addition to what I heard in your video I would check the few things I listed as well.
I still have a sneaking suspicion that it may be a Soft 15 thing. Has anyone had any experience with a ATI 9250 and Soft 15? Maybe It has something to do with the fact that it displays 2 generic monitors rather than one? I still think its a signal thing, if only because When I try and boot using the Jpac WITHOUT the jumper on the 31khz prongs, it wont give me any signal whatsoever. Anyone have any ideas?

Try something like 640 x 240 or 560 x 240 .
Maybe also try around 50Hz on the refresh rate.

Those are recommended resolution and refresh rate for the 25K7191? :dunno
I will give this a shot. Im hoping this combined with the new remote board, and a new VGA cable will help resolve my issue... :dizzy:

Well, when trying to troubleshoot sync issues it's always a good idea to start in the neighborhood of what the monitor may actually be capable of. ;)
(and then play with it from there)

Well during the boot sequence and loading screen the picture is split which is to be expected with the use of my Jpac and still has the same flickering and scrolling issues as when windows loads, however once windows loads everything is very much squished to the right hand side of the screen. I am still unable to fix this, and am about to just give up on it all together to be honest. I am out of ideas and patience to resolve what is seemingly a simple issue.

Your pretty much going to have to strip it down and try it on different computers. If you get the same result on 2 different computers using different graph cards and different drivers it is pretty safe to say that the monitor is no good.
If it works then you can start looking at your computer hardware/ Graphics card might not be compatible or the drivers might be not be correct. Try swapping out the Graphics card with the machine that worked.
You first need to identify were the problem is coming from before you can fix the issue.
