Go to Options->Viewer, then check your ctrlr file. Make sure the keycodes on your labels match the keycodes in your ctrlr file. If you didn't put in a ctrlr file entry in the viewer options, then that may be your problem.
I don't think there is an official forum per se, but you could probably post something on fe.donkeyfly.com if you can't figure it out yourself.

I checked in there and just put in xarcade since that's one that's in my ctrlr folder. I opened up that file and saw stuff like KEYCODE_LCONTROL and that's the same thing on the label when I open the default layout in Johnny5.
No idea why it's not working. *sigh*

I had the same issue when using mame64.exe, but my ini was named mame.ini.
Mame ran fine but J5 had the exact problem you're describing.
I copied mame.ini to mame64.ini and my labels appeared.
J5 seems to require that your exe and ini file have the same name.

Nope. I'm just using Mame103 (mame.exe) and Mamewah as the FE. The only mame.ini file I have is inside the Mamewah/INI folder. Is there supposed to be one in the main Mame folder too? Maybe I'm missing a file somehow.
Extreme8:
Create a mame.ini file and give it a shot.
type mame -createconfig and it'll make the mame.ini file for you.
I'm a long way from a J5 expert, but I did get it working. The .ini file was the hangup for me.

Well I had it create the file but it didn't help just by doing that. Are there any settings in there I need to point anywhere special? hmm.
sharkus:
Have you defined a ctrlr.cfg file that has all of the keys on your control panel defined? You must do this since the later versions of mame do not create a default .cfg.
I have a sample for the ipac2/minipac on this thread:
http://forum.arcadecontrols.com/index.php?topic=49115.0
Also, once you create your .cfg file don't forget to update your mame.ini file to use it!
Good luck,
Sharkus
