Rarely used.

Depends - First off, the classic Neo-Geo was set up to be
   2 3 4
1

With 1 being primary and pressed with the thumb.  That thumb button becomes important for that.

(and a lot of people, including me, used to think the button layout was Index to pinky).

I think some people may be playing the games -

With the thumb button activating button 4.  It is less critical there.

I know the baseball games use Button 4 for Time out.  It is also used in NG fighters, but I don't play those.

I would think Stargate - maybe Space Duel.

Your thumb working the 4th neogeo button. It's comfortable enough to use for extended periods.

The only problem I see with this is that Mortal Kombat 3 uses the 7th button for "Run".  If that button is hard-wired to the same input as another you've got a problem.  I feel that the best way to handle it to give each button a unique input and just reassign them as necessary (it only takes a minute and you only have to do it once for those games that require it).
