Looks kinda like Verve or Tube, but I can't find the exact cut.

The font is called Bodiac. I figured this out by searching through countless fonts online until I matched it up.
Also, a question for Motorfish - I have made a number of mods to this (including breaking things out into more layers and leaving things vector based rather than rasterizing them) and wanted to share. Would this be okay with you?
I didn't want to overstep my bounds by offering your original artwork in a modified version unless you were okay with it.
