Joe from Game Sack YouTube channel showed off the original ad in a magazine. The model's name is Brandy Ledford. The segment is during the ending skit.

The more tame version is in the second photo, from Compute! magazine. The guy isn't fighting the cock monster from Cyber Lip. He's playing something else that I don't recognize.

Ninja Combat, the HUD at the bottom is a dead giveaway.  Joe from Game Sack is great, the other guy is a drag.  I've been mentioned on Game Sack a few times. Jealous? No? Didn't think so =(
