Here's what I have. I have one cord into this but I have two usbs up top where you have one.

I have two USB inputs on the top of mine.

Do you see that wire from your power supply to your i/o card and how it's connected to one of the side ports?  Mine is powered with a smaller connector on the back of the i/o card.  I wonder if that's causing your issue?  Pull that larger molex connector out and place the smaller one from the PSU to the back and see if that may work?

The one on the back is larger. Doesn't look the same.

Hmmmm, do you have anything you can plug into that port?

Slimonce:
No. The light on the back comes on when I have it on. Green light next to big port.
