I have a D9200 monitor and a 4350 Radeon Card.. Where to start?

I'm upgrading my mame cab and I ordered in a 4350 Radeon card to try. Where is a good starting place to look for setting up 15/24/31.5 modes? (If the card works I'm keeping it, if not I guess I'll get a new arcade vga.)

Should work great with Windows as long as it's not Windows 7, should work even better with my Linux live CD especially if you don't have XP64 and want 64 bit :-).  In Windows 7 you are pretty much stuck with using an ArcadeVGA3000 card there, but there's other options which are cheaper too (plus more flexible, Linux can spot on the vsync and modelines per game perfectly, at least as close as the monitor can handle).  At least with the Live CD you can boot it and try it without much trouble or installing anything, and trying out the windows builds and custom radeon drivers at http://mame.groovy.org I and Calamity have worked on might be of interest to play with and compare to see which you like better.

What I was looking for is instructions / hints / a walkthru for setup.

I'll be running XP 32bit.

I remember when I setup my current arcadevga setup, I used a utility to setup the correct res for each game automatically. I seem to remember that the 15k modes are addressed as 1 scanline extra. (320x201?) Does soft 15k need anything like this?

I'm starting from scratch, I might keep mamewah, unless something much nicer is around now.

I've got a full set for 1.41u3, but I'm trying to get the 1.42 update.

If you're running a cab, do yourself a favor and at least try the groovymame linux live cd.  I had XP64, soft 15KHZ, and a ATI4350 and had decent success, but it really, really, really doesn't compare to the live cd.  With XP, you can dial in the resolutions, but the refresh rates can cause tears, jittering, etc.  The XP version of groovymame is very nice (better than soft15KHZ if you have Calamity's drivers), but the linux distro is way better.  With the live cd, the resolutions are spot on and the refresh rates are spot on...especially since you have a D9200.  So you absolutely get the best of both worlds.

I've got the groovymame linux setup as a hd install and could not be happier.  After a couple of evenings, I pulled out the XP hard drive and it's being used elsewhere.  I had Hyperspin as a frontend on my XP setup....but I don't miss it anymore as I customized the wahcade layout to my liking.
