I don't have the stick, as this is a question from a friend, but I'm trying to come up with possible reasons for the problem.
Apparently, the actuator is off-center, and is significantly closer to two of the microswitches than the others (apparently even touching one, though not enough to engage the switch). I'm trying to figure out what could cause it to get off-centered like that. My first guess was that the plastic sleeve inside the stick was off center, but I'm not sure how you'd fix that. It just rests on the spring in there, doesn't it?
Any other ideas about what could cause it so I can give him a laundry list of things to check?

I had the same problem I just loosened up two of the micro switches that were closer to the actuator & shift them outwards a slight bit & it worked out for me :P

Guess I should have also asked if the "stick" itself is centering up properly?
I've seen people put that part #95-0199-10 in backwards and it will cause this sorta problem. (the stick is usually feels kinda clunky as well)

Without seeing it, I'm inclined to believe that this is one of those "non-issues". These types of sticks have a sleeve part for the shaft that has some lateral motion. It's necessary in order to allow the pivot cylinder to have space to tilt.
To see what I am referring to, just "push" the stick in any direction, but don't tilt it. You should see the proximity of the actuator to the switches change. This is normal for this type of design and shouldn't affect gameplay.
