Title: Common problems in Arcade VGA Cards - Can they be fixed?

When an Arcade VGA Card randomly dies - what are the most common causes? And can they be fixed? And what does it entail and how easy is it?

once that dies, give up and run soft 15khz

I'd bet the same issues for any video card, generally.
