Looks like this background (http://www.localarcade.com/arcade_art/r-homebrew-9-fusion-explosion-marquee-(background)-296.htm) and then a custom MameWAH skin.  It looks like the download is corrupted though.  Can't seem to find another download for it...

I saw it on a cabinet that Knievel did.  Not sure if he made it or just got the skin somewhere else.  Emailed and pm'd him, but no response...

If you read Knievel's thread I believe he does his own custom skins on all his cabs.
