I have already cut wire #6 making it output 15.75 kHz from the vga connection. Just add a hacked vga cable (not included) and you too can play dreamcast games on your arcade monitor and in your cabinet.
I have two, this Naki one and a Madness Gameware. I'm keeping the Madness. Bought this one "new in box" about a month ago see (http://cgi.ebay.com/ws/eBayISAPI.dll?ViewItem&category=62053&item=8248811434)
Asking $18 shipped in the US.
PM me if interested.

Is their a reason you like this one less?
also does the s-video output still work at the same time?

Right, I should have mentioned that. The only difference is that the Madness has volume control and the Naki does not. Volume must be controled via and enternal amp on the Naki box. Video quality, IMO, was the same between the two.
s-video still works, yes. As with all of these units, you must switch between vga or tv output.
