This is a control panel for a Dynamo HS-5 cabinet. This control panel is model # 991501301 Rev.1, made by Dynamo Corporation. It is usually used for making a 4-player control panel game out of a two-player control panel game.   It may be a little cramped for 4 controllers w/ 6
