That looks just like one I have. I've still not been able to determine the exact model
but the manual for a SI-327 ( link)

is very, very close.
Mine was dead and I used that manual for the repair.
It is either a Kortek or a sharper image monitor.
Look at the korteks on this monitor id page.

Also, remember that the adjustment board can plug directly into the monitor board or have a cable to a remote location like yours.

Definitely a Kortek (probably sold by either Betson-Imperial a.k.a. Imperial International or Sharp Image).
Edit:
Try Zanen Electronics capkit #501 or 520. Either one should work.

Thanks for the help guys. That 26" Kortek looks about at close as I am going to get. I've got the Si-327 manual you suggested I download.
Do you think a cap kit for that model would be my best bet rather than chasing all the parts up individually?
Also can you recommend someone to get the capkit through besides JOMAC, or did you go through them?

I'll give the guy from Zanen a call but are you sure about those numbers for the kit. Looking at the list of kits, I would have thought the #608 would have been the most likely. Not that I know much at all about it, but the board looks to be almost identical to that model (Si-327, sharp image)

I've never seen a Sharp Image SI327, but the picture of that chassis is very close to what I have worked on previously and kit 501 matched it.
