Star Trek uses 4?  But if you don't play that game, I can't think of any others that use more than 3.

Wacko uses both a trackball and a 4-way.  Tron has a 8-way triggerstick.

Related question - what games had more than one spinner or trackball in a single CP?  (i.e. simultaneous multiplayer) Marble Madness had 2 trackballs, and there was a 4 trackball version of Atari Football.

Atari's Sprint series technically used spinners as steering wheels.  Super Sprint (Championship?) had three spinners.  Of course, Sprint 8 had 8, but obviously not on one panel.  Blasteroids was a dual spinner game.  Can't think of any others off the top of my head.
