I had the problem with a rolling image when running Pacman rotated on a horizontal tv, so I switched the monitor type from arcade_15 to ntsc. Now switchres runs Pacman at 2560x480i to fit it on a horizontal screen. There is no rolling, but the game now runs at 200% speed. If I use -norotate and run it like I would on a vertical monitor it uses 2560x240 and runs at normal speed, so it appears to be an issue using interlaced video. Is there a way around this?

Please post a log of pacman when it runs at 200%, I need to check your settings.

Ok this looks like a genuine problem, I'll need to check it here. By now, I think you can circunvent the problem by setting frame delay above 0, you can set it from the ui.

Sounds like what I experience with a few games, like Xexex, until I turn frame_delay on the game runs like about 200% indeed.
Dunno if related but it could maybe: after you've just set vsync_offset for a game, then go back to the menu and select a different game; the previously set offset value is carried over, like it stuck to the slider and remains until you reset it or set a new value.
Specific ini files experienced a similar issue until 203 where it wasz fixed after I reported it.
These are problems I've mentioned not long ago, and BTW I also experience random crashes on games launch from time to time, maybe it is related.
PS: if you always use command line instead of the UI it is possible you don't experience such issues.
