Which is the proper way to configure vmmaker for a vertical monitor? Under Monitor Settings > Monitor Rotation?
If so, is Vertical (fixed), or Rotating to the right the best option?

I think the ¨fixed¨option is for when your displaying vertical games on a horizontal screen.
On my vertical setup in VMM i use rotating to the left,and in MAME.ini ¨aspect 3:4¨

Yes, the rotating options are preferred. By the way, starting with GM 228, rotation options are no longer handled by GM, so you should leave this as default "horizontal" orientation, and instead, select the correct -autorol or -autoror option in mame.ini.

One more quick question about vertical setup if i may.
This might be a stupid question,but does it make any difference which orientation you have windows when you generate new modes in VMM? The reason i ask is its easier to navigate the settings in horizontal,then rotate the desktop later.

As far as I remember the "desktop rotates too" switch only affected the settings VMMaker exports to GroovyMAME, but not the video modes itself. VMMaker doesn't check Windows orientation either (that's the point of the mentioned switch), so it makes no difference to the modes you generate.
On the other hand, the monitor orientation option (horizontal, vertical, rotating) does have an effect on the way the modes are calculated.

Thanks for clearing that up.Its easier to navigate windows in horizontal mode with my head to one side than it is in vertical orientation. ;D
