Astro City Control Panel - 2 player, 3 buttons each. (2L6B) $50 shipped
Top overlay is original gloss, in decent condition, but there is a slight bit of rust under the left corner that is causing a small bulge. Not too noticeable when it's installed though.
Rust has been cleaned up a bit on the backside.

Turns out that capture card doesn't do 1080p @60fps. Here's what it supports:
Video Format: 1080i50, 1080i59.94, 1080i60, 1080p23.98, 1080p24, 1080p25, 1080p29.97, 1080p30, 720p50, 720p59.94, 720p60, 625i 50 PAL, and 525i59.94 NTSC
HDMI formats: 525 NTSC, 625 PAL, 720HD, and 1080HD switchable
I've cut down the price by half.
