I have an iPac4 (PS/2, not USB) that I purchased from Ultimarc a few years back that I have installed into a mame controller box. It worked great early on, but I hadn't used it much in the past several months and now it gives me the flashing LED that then goes out. I saw on Ultimarc's website that this is a 'failure' but it doesn't go on to say why or what troubleshooting to do. I'm running Win 10 now and may not have tried to use the iPac since upgrading, so I can't tell if that's the issue or not. (I do get a solid LED if plugging into the passthrough).
WinIpac doesn't recognize the iPac at all. Any ideas? Is there a forum topic somewhere that addresses this?

Try removeing it from the device manager and re-adding it?

I have the same issue but mine was purchased around 2007. So it doesn't get recognized by Windows 7 anymore. So I have to manually unplug it and plug it back in. Now I am not saying that will work for you but just understand that there are issues regarding upgraded windows operating systems.
I would definitely contact Andy over there and he can let you know what to do.
Also check to see if there is an update on the firmware on the ultimarc website.

It will be listed under the keyboards I think.

All I see are two instances of "HID Keyboard Device" and neither seem to have any identifying info that would tell me which (if either) is the iPac.

One will be the IPAC, One will be a regular KB (I presume) It wont hurt removing both should just auto reinstall when you plug them in to the PC.
