My cabinet uses an old SlickStick for the CP, with a ps2 connector for the keyboard encoder.
The pc died, and the new one only has USB. What would be the best recommended ps2/usb adapter? I've had mixed success with adapters for various purposes in the past.

The problem you run into is that USB and PS/2 are not compatible protocols. There are two different types of converters. One is just a straight through cable - these are used when the device already knows both protocols and adjust internally. The other type has a little block in the middle - those actually do the conversion in the cable and should work on all devices.
If you are not sure if your device auto-detects, then spend the extra couple bucks and get the other kind. They are only about $7.

Something like that. Make sure you get the correct genders on the connections. I use one of those on a Sparkfun trackball, works great. Got mine from Fry's Electronics though since I'm impatient and didn't want to wait on shipping.
Also a small caveat. I don't know how these adapter deal with the multi-button limitations. I can never remember the specifics of that though, sorry.

Did they use iPacs in those? I can't remember for sure, but I thought they did. If so, take a look if it says USB next to the PS2 connection. If it does, then you may need to get the cable from ultimarc. I bought one of theirs after failing with one like you showed.
