I've installed two UltraStik 360's into my cabinet and the USB-mode works fine and Windows detects the joysticks.
However, I've attached the 10-pin header to the joystick, and then connected the following wires:
USB = (connected to PC)
BLACK = IPAC GND
ORANGE = P2 - Right
YELLOW = P2 - Left
GREEN = P2 - Up
RED = P2 - Down
However, the IPAC doesn't detect any inputs from the joystick. Do I have to enable the "Output Mode" of the U360 joystick somehow?
I am also wondering if I could daisy-chain the four direction wires to my dedicated 4-way joystick (so both joysticks could be used in digital/keyboard mode if I wanted)

Do you have 5 volts wired to it.

I don't think it's needed because it's powered via USB, right?

I may be wrong, as I've never used one but I believe you will need to wire up 5 volts to the output wiring harness.

Output mode is autodetected. You don't need 5V if connected to USB. I use my U360 this way without any problem.
