I'm very new to this.
Have bought a machine with about 14,000 roms in the mame rom folder but when mala runs mame, only about 4000 roms are showing.
What do I need to do to show them all?

Have you messed with the configuration at all? or did you just buy the machine as is and plug it in? Chances are with 14000 roms, there are a lot of doubles and non working games that the PO removed from the list. My machine personally I have been spending a lot of time going through the list removing things like that.
If you really want them all on there, (I honestly wouldn't, You won't have time to play them all and it makes it a PITA to find the games you do want to play) You can right click in mala and go under the configurations and there is an option to add games to game list.

I'm not super familiar with Mala but I'm guessing there will be a games list folder and that the mame list has been shrunk using something like Romlister. I would do a quick google search for Mala romlister and see if the location of the file is revealed.

This is how I've bought the machine, only had it two weeks and trying to sort through it.
I've not really touched the configuration at all.

Probably doesn't have clones and non-working games enabled.
Right click with the mouse to enter the config menu.

By the time I weed out the Mahjong, gambling, adult, non-working, and unwanted clones, I end up with around 2,500-2,800 games (don't remember the exact number right now).
At 14,000 you're probably going to have a list that is 80% crap you don't want anyway.
There are some worthy clones (pacman is a clone of puckman), but I don't like them all showing.
Once you get a little more familiar with all this emulator stuff, check out romlister to weed out stuff you don't want.

You're probably right, there seems to be lots I need to get rid of.
The game game I really wanted was defender which doesn't play right.
Moving joystick left doesn't reverse the spacecraft and moving joystick right doesn't
Use the thrust. Both options are on the buttons so its not right I think

IIRC on a real defender cabinet, there are dedicated buttons for reverse and thrust.

You remember correctly. The original joystick was only 2-way (up/down).
bhamoggy, you can remap the reverse/thrust to the joystick in the options but I'm guessing it will not be "relative" (i.e. if you map "thrust" to right, it'll always be right even if you're going left.) You're going to have to do it old school.

You're right, tried remapping but not good so have to go old school
