I'm fixing a mame machine for a guy who knows nothing about them, he bought it from someone then contacted me based on a friends suggestion. From what I can tell, it has a J-Pac and a regular arcade monitor. If the computer is plugged into a pc monitor is fine, when it goes though the J-Pac it looks like this:

So does this appear to be a J-Pac issue?

Are you running soft15khz?

Not that I'm aware of, i've not check out the PC at all. All I seen was the computer booting up then the screen being garbled.

It's amazing software and I'm fairly certain it's essential for having a computer display on a standard arcade monitor. I think your other option would be to use one of these: http://www.ultimarc.com/avgainf.html (http://www.ultimarc.com/avgainf.html)
I've never used one, but I've heard they work well.
My advice would be:
1. Do a bit of reading on both so you feel like you have a good understanding of them.
2. If you use soft15khz make sure you have a compatible video card.
3. If you use arcadevga make sure you have the appropriate slots on your motherboard.
4. Make sure the electrical wiring is appropriate as far as the isolation transformer for the monitor and taking care of the wiring to the switch power supply. Again, I'm anything but an expert, but I've heard that it can be bad to leave the switch power supply powered (I've also heard that it doesn't matter)...

your going to have to change the resolution that is output by the computer to match the monitors capabilities.
looks like your trying to display a high resolution video signal on a monitor that only supports low or possibly medium resolution.
soft15k will work for you, but it's best to find out what the capabilities of the monitor are before you go any further. if you take some photos of the monitors electronics or can find a model number and post it here, we can tell you what it can display, and tell you how to configure the computer to output a good signal.

thats a wellsgardner k7000
standard res only (15k)
you will have to hook up your computer monitor and change the resolution to something in the neighborhood of 320x200 @ 60hz
then swap to the arcade monitor.

If you have an arcade VGA it will already be able to do it, otherwise, yes you will have to install soft15k

Ok. It's currently just running a Radeon 8500 card. Story goes the Mame pc crashed, guy took it to a local PC shop who didn't know it was hooked to an arcade monitor so there was no software installed apart from Mala. Will I need to install catalyst then soft15?

Sounds right. Course it doesn't always happen that way. But give it a shot.
Sometimes you have to install an old version of catalyst to get things working, sometimes only the newest/greatest/beta drivers will work.
Be prepared to try a few times.

I have the same screen.
Windows resolution is set to 800x600 and running soft15khz. Works like a charm once in windows (XP)
You do need to load the catalyst drivers first, but those monitors will run 800x600 when running soft15khz. Best to get the machine all up and running on a normal screen (non arcade) then, install soft15khz, and move it to the arcade at that point. Boot up will still look bad, but, once in windows it will be better.
If still rolling, then it could be the vertical/horizontal hold pots need to be played with until a perfect screen is seen. I have had to do this on mine when I changed resolution.
