MAMEHOOKER Runtime error "9"; SubScript out of range

Subscript out of range is a generic windows error in regards to reading beyond the upward limit of a variable or array or something else read into the app.   It could be a million things, but it's often caused in mamehooker by either an improperly registered ocx (think ledwiz) or a scripting error.   I can't help your anymore than that, but for future reference it is never necessary to reinstall windows or anything that drastic to get mamehooker running.   It's just an app dude, make sure the app has everything it needs and you'll be fine.
