Winipac won't map keys properly

Hopefully I'll get this kinda close. I've had a MAME that I bought back in 2013. Worked well up until the other day. I've done no update, changed NOTHING. Its not even connected to a network. After booting Millipede in Hyperspin, the fire button just stopped working. I've tried re-mapping the button but I'm not getting any success. I probably won't way it correctly but I can press the keyboard and see the button "working" on the screen in Winpac. I can even fire using the keyboard. However, I can't get the button to map so that I can fire from the previously assigned button. I tried dis and re-enabling the driver in Control Panel and disconnecting and reconnecting the USB to see if that would work. I have a video here on youtube that visually shows the challenge in case I'm not writing about it properly.

Couldn't fully watch the video. But if you start notepad or anything you can type some text in, do your buttons do anything?
