Tried to search on the FCC number and this came up: http://vectorgaming.proboards.com/thread/972/hardware-tests-wip?page=1#

It's in the signature of one of the posters.

It also has an entry at KLOV. http://www.arcade-museum.com/game_detail.php?game_id=18611

I have to say this is the first pre-90's or even possibly pre-00's production cabinet I've seen with *cade as part of the title.
